  • Dear Parent/Guardian:

    Big Brothers Big Sisters of Guelph (BBBSG) in partnership with the Wellington Catholic District School Board (WCDSB) and Upper Grand District School Board (UGDSB), is pleased to offer the Adult In-School Mentoring Program.

    In the Adult In-School Mentoring program, a deserving child/youth is matched with an Adult Mentor. They will spend an hour each week together on a one-on-one basis for the school year, during school hours, and only on school property. This is not an academic program, but an activity-driven program to encourage a friendship between the Mentor and Mentee (child/youth). They may play games together, enjoy sports, cook and bake (where permitted), take on crafts, or read together. The objectives of this program are to promote social and emotional skills, mental health and wellbeing, and educational engagement through the development of a caring relationship with a Mentor.

    The Mentors are adult volunteers from the local area. They have been recruited, screened, and trained by Big Brothers Big Sisters of Guelph.

    If you are interested in this program for your child, please complete the enclosed forms and return it as soon as possible. Completing these forms is the first step in the matching process. This is how we begin getting to know you to see if we have someone who would be a compatible fit for your child.

    Although you do not have to be involved in any particular way, the following options are available to you.

  • Every child in the program participates in pre-match training which is located on our website here: https://guelph.bigbrothersbigsisters.ca/what-we-do/child-safety-training/. If you are interested in learning more about this training, please watch the video and/or contact the caseworker.

  • Signing this form indicates an interest in the program, as well as an understanding that a match can only be made if a compatible mentor is available.

  • Informed Consent (Site-Based 1:1 Program) - Parent

  • I hereby make formal application to Big Brothers Big Sisters of Guelph (BBBSG) to make available their service to my child. It is my understanding that the intention of BBBSG is to match a responsible male/female adult, (minimum 18 years old, however, where appropriate supervision takes place, the volunteer may be younger), with my child for the purposes of shared activities, friendship and support. I understand that all efforts will be made to select a Mentor who is compatible with my child.

    In consideration for this service and other valuable consideration provided to my child by Big Brothers Big Sisters of Guelph, I release the agency of all responsibilities and liabilities in connection to their services provided in good faith, to myself or my child. I permit the agency to release any relevant information, including my personal information, to Big Brothers Big Sisters of Canada and their insurers, as may be appropriate in connection with any legal proceeding, inquiry or risk thereof.

    I understand that the collection of personal information about me or my child will be held in strict confidence and is to be used solely for the purposes of administering the program. I further agree that information about my child may be shared, at the discretion of Big Brothers Big Sisters of Guelph, with my child’s Mentor so that my child’s needs in a Mentoring relationship may be best met.

    I understand that I am under no obligation to accept a Mentor for my child, that the Agency is under no obligation to provide my child with a Mentor and that this application is the property of Big Brothers Big Sisters of Guelph. I also agree that my child will participate in the Pre- Match Training Program administered by Big Brothers Big Sisters of Guelph and the school liaison.
